Midwest Dental Terminates Contract

Dear Participant:

Effective September 30, 2004, Midwest Dental will no longer provide free yearly exams and cleanings to Wisconsin Carpenters' Health Fund Participants and dependents, as they are terminating their contract with Wisconsin Carpenters' Health Fund as of that date.

You may continue to use Midwest Dental and benefits will be processed under your dental benefits, subject to the calendar year deductible and benefit maximum.
